HIPAA /Telehealth Data Use Policy

1. What We Collect

• Your personal details (like name, date of birth, insurance info)
• Your health information (symptoms, diagnoses, treatment plans)
• Technical details (like device type or connection logs for security)

2. How We Use It

• To provide you with safe, effective medical care
• To coordinate with other providers when needed
• To process insurance and billing
• To improve our services and meet legal requirements

We never sell your information or use it for marketing without your permission.

3. Who We Share With

• Other doctors, pharmacies, or labs involved in your care (with your consent)
• Your insurance company (only what’s needed for claims)
• Vermont’s Health Information Exchange (VHIE), unless you choose to opt out
• Public health agencies if required by law

4. Your Rights

• You can see and request a copy of your records
• You can ask us to correct mistakes in your records
• You can limit certain types of sharing when the law allows
• You can opt out of the VHIE if you don’t want your data shared there

5. Security

• All telehealth visits are encrypted
• Records are stored securely and only accessed by authorized staff
• We follow HIPAA and Vermont privacy laws
