At Fasci Family Telehealth and Wellness, your privacy and trust are very important to us. Here’s how we handle your information when you use our telehealth services in Vermont:
1. What We Collect
- Your personal details (like name, date of birth, insurance info)
- Your health information (symptoms, diagnoses, treatment plans)
- Technical details (like device type or connection logs for security)
2. How We Use It
- To provide you with safe, effective medical care
- To coordinate with other providers when needed
- To process insurance and billing
- To improve our services and meet legal requirements
We never sell your information or use it for marketing without your permission.
3. Who We Share With
- Other doctors, pharmacies, or labs involved in your care (with your consent)
- Your insurance company (only what’s needed for claims)
- Vermont’s Health Information Exchange (VHIE), unless you choose to opt out
- Public health agencies if required by law

- You can see and request a copy of your records
